Breast reduction surgery is often perceived as a procedure to reduce size, but one of the most critical factors determining aesthetic outcomes is the postoperative position of the nipple.

Dr. Jinhee Hwang, Director of Humanity Plastic Surgery, drew attention at the Academic Congress of the Korean Society of Plastic and Reconstructive Surgeons (APS 2026), where she delivered a lecture on how nipple position changes over time after breast reduction surgery and how these changes can be predicted in advance to achieve improved outcomes.

APS is an international academic conference held annually in Korea, where board-certified plastic surgeons from Korea and around the world gather to share advanced surgical techniques and clinical experience, engaging in in-depth discussions.

As a speaker in the breast surgery session, Dr. Hwang explained, “Although we meticulously design nipple position before surgery, breast tissue continues to change afterward, and the nipple does not always remain where it was initially placed.”

According to her presentation, nipple position may shift upward over time depending on the surgical method and tissue characteristics. In other cases, elongation of the lower breast may create the appearance of a higher nipple position.

Dr. Hwang emphasized that anticipating these changes and incorporating them into surgical planning is essential for achieving stable, long-term aesthetic outcomes. Rather than relying solely on preoperative measurements, surgeons should adjust nipple positioning in advance based on expected postoperative changes.

She also stressed the importance of avoiding overcorrection. “A nipple positioned too high is difficult to revise,” she noted, highlighting the need for a systematic design approach that accounts for this risk from the outset.

Through her presentation, Dr. Hwang underscored the importance of a predictive design concept in breast reduction surgery—one that considers postoperative tissue dynamics rather than focusing solely on surgical technique.

Her lecture attracted significant attention from both international and Korean plastic surgeons, as it provided practical, clinically applicable guidelines.
