워싱턴 DC 를 포함한 미국의 모든 주에서는 기본 학력과 주 자격 시험을 통과한 카이로프락터들에게 라이센스를 발급하여 practice 할 수 있도록 하고 있다. 라이센스를 발급받은 주에서 만 기본적으로 일하게 되어 있지만 주에 따라서 타주에서 받은 라이센스가 자신의 주에서 요구하는 기본 조건을 충족 할 경우 추가 시험없이 라이센스를 허가하기도 한다.
앞서 말한대로 최소 2년의 학부 교육과 4년간의 카이로 프랙틱 스쿨을 졸업한 자에게 시험자격을 부여하고 있으나 Florida Kansas Montana Rhode Island Wisconsin 그리고 U.S. Virgin Island의 경우 반듯이 4년제 학사 학위와 DC 학위를 요구하고 있다. 이에따라 대부분의 카이로 프락틱 스쿨도 학생들에게 학사 학위를 입학당시 요구하거나 재학중 학사학위를 동시에 따도록 하고있다.
대부분의 주의 board는 the National Board of Chiropractic Examiners 에서 주관하는 4 부분 (four part test) 국가 자격고시를 전부 혹은 일부분 인정하는데 주법에 의하여 주자격 시험 으로 인정하여 라이센스를 준다. 라이센스를 취득한 후에도 지속적으로 공부를 해야만 자겻이 유지된다. 카이로프락틱 협회 중에는 회원들에게 orthopedics neurology sports injuries occupational 과 industrial health nutrition diagnostic imaging thermography internal disorders 분야에 관하여 소정의 교육을 제공한 후 수료시 "diplomate" 자격증을 제공하기도 한다.
